Chronic Inflammation: Silent Driver of Disease, Fatigue and Aging
In today’s world, many individuals are unknowingly living with an internal fire. It often does not cause pain (yet) or fever, nor does it trigger a red alert from your doctor. Yet, this low-grade, persistent biological inflammation is silently fuelling everything from fatigue and brain fog to heart disease, insulin resistance and neurodegeneration.
This condition is known as chronic inflammation, and it is now widely recognised in scientific literature as a root cause of many modern chronic diseases. Looking at the root of why the body is chronically inflamed is essential for anyone seeking to not only extend their life span, but also ‘put life into those years’ – ie to enhance the quality of those years. We also call this ‘health span’.
What is Inflammation, and When Does it Become a Problem?
Inflammation is not inherently bad. In fact, acute inflammation is a protective response designed to eliminate pathogens and promote tissue repair. It is what causes swelling after an ankle sprain or helps the body fight off a viral infection. It is crucial for survival.
However, chronic inflammation is a different entity altogether. It occurs when the immune system remains activated long-term and begins to damage healthy tissues. This inflammatory activity becomes misdirected, creating a constant release of cytokines and free radicals that degrade cells, hormones, and organs over time.
Researchers have coined the term inflammaging — a persistent, low-grade inflammatory state associated with the aging process. It is now implicated in virtually all major age-related diseases: cardiovascular disease, Alzheimer’s, Type 2 diabetes, osteoporosis, and many forms of cancer (R).
Signs You May Be Living with Chronic Inflammation
Because chronic inflammation is not always overtly symptomatic, many individuals live with it unknowingly. Common manifestations include:
- Unexplained fatigue or low energy
- Joint stiffness or muscle aches
- Cognitive decline or memory lapses
- Insulin resistance or weight gain
- Belly fat despite being skinny – aka ‘skinny fat’
- Eczema, acne and other skin conditions
- Recurrent infections or sluggish immune responses
- Hormonal imbalances such as manifested in endometriosis and PCOS
- Mood changes or increased anxiety
If you recognise several of these signs, it may be worth exploring targeted lab testing to identify underlying inflammatory activity.
The Root Causes of Chronic Inflammation
The development of chronic inflammation is rarely due to one single factor. Instead, it results from an interplay of genetic predispositions (R), lifestyle patterns, immune dysfunction (R) and environmental exposures (R). Some of the most common underlying trigger include:
- Hyperglycaemia and Metabolic Dysfunction
Even short-term elevations in blood glucose can activate inflammatory gene expression in blood vessels — a phenomenon known as metabolic memory. Researchers found that vascular inflammation persists for days following a single episode of hyperglycaemia (R).
Furthermore, high blood sugar leads to the formation of advanced glycation end-products (AGEs), which bind to receptors on immune cells and promote inflammation and oxidative stress. These literally ‘age’ us from the inside out (R).
- Visceral Fat as an Inflammatory Organ
Excess fat around the abdominal organs (visceral fat) is not just a passive energy store — it is metabolically active and pro-inflammatory. It secretes immune messenger chemicals (cytokines), which perpetuate insulin resistance and systemic inflammation (R).
The NLRP3 inflammasome, highly expressed in visceral fat, contributes to both metabolic and cognitive impairment by activating inflammatory signalling and promoting immune cell activation in the brain (R).
- Latent Infections, the Gut Microbiome, and Immune Dysregulation
Persistent viral infections, particularly cytomegalovirus (CMV), are known to chronically stimulate the immune system, leading to elevated TNF-α and reactivation of latent viruses — a self-perpetuating inflammatory loop (R).
Most of my readers are by now familiar with the concept of the gut microbiome. We all have bacteria in our gut, which are needed for health. However, if we have ‘the wrong type’, or not enough of the ‘good ones’, this can activate the immune system to overproduce inflammatory chemicals. Common patterns include SIBO (small intestinal bacterial overgrowth, where bacteria take over the small intestines and cause bloating and discomfort), SIFO (small intestinal fungal overgrowth), H.pylori ( a bacteria mostly located in the stomach, which can contribute to stomach ulcers and reflux, all the way to the development of stomach cancers), parasites and more.
If our gut microbiome is ‘off’, this then commonly leads to a ‘leaky gut’, a phenomenon where the gut lining, which generally should be nice and tight, and only allow tiny particles into the blood stream, becomes more permeable (aka ‘leaky’). This can then not only allow parts of the gut bacteria to pass into the blood stream, triggering our immune system, but also foods in a different shape to what the immune system is used to.
Often this then triggers ‘food intolerances’ to healthy foods, such as eggs, almonds, or yeast. Once the immune system has mistakenly classified these foods as an ‘invader’, it now mounts a response every time one consumes them – in the form of chronic, low grade inflammation (R).
- Oxidative Stress and Mitochondrial Damage
Inflammation and oxidative stress are intimately linked. Free radicals, generated during chronic inflammation, but also from other factors such as environmental pollution, cigarette smoke, excess alcohol use and more, damage mitochondrial DNA, further activating inflammatory pathways, making this a self-perpetuating cycle.
This oxidative-inflammation loop is thought to be a key mechanism in chronic fatigue, neurodegeneration, and cardiometabolic syndrome (R).
- Psychosocial Stress
While acute stress is generally considered anti-inflammatory, the opposite is true for chronic psychological stress.
Research suggests that chronic stress (and high cortisol levels) eventually make the cells less responsive to the anti-inflammatory action, thereby increasing overall inflammation in the body, while at the same time suppressing the immune system’s ability to fight infections, making this a double whammy.
There is a strong link between chronic stress and diseases which have a chronic inflammatory component,, such as cancers, autoimmune disease, and more.
Many of you might know a person that was doing OK, until a stressful life event tipped their health over the edge (R).
Functional Medicine Strategies to Reverse Chronic Inflammation
- Comprehensive Inflammatory Testing
The core tenet of Functional Medicine is this: we test, not guess.
By identifying the underlying causes and biochemical pathways driving inflammation in the individual, we can create a personalised plan to correct it.
Some of the key tests I commonly use in my practise to assess inflammatory drivers include:
- Gut microbiome testing
- Other chronic infections such as mold and candida
- Viral panels
- Hidden food intolerances
- Heavy metal load
- Adrenal stress panel
- Genetic SNP testing for genetic predispositions
- Nutrient levels needed to quench inflammation and free radical damage (such as methylation co-factors folate and B12, omega 3s and more)
- Blood sugar balance
- Nutritional and Lifestyle Interventions
a) Anti-inflammatory Diet
A diet rich in whole, unprocessed foods with a high content of fibre, polyphenols and omega-3 fatty acids has been shown to significantly reduce inflammatory biomarkers. Key components include:
- Fibre-rich foods: Legumes, oats, flaxseeds, vegetables (R)
- Polyphenols: Berries, green tea, turmeric (R), extra virgin olive oil
- PUFAs: Salmon, sardines, flax oil, walnuts (R, R, R)
- Anti-glycation agents: Cinnamon, alpha-lipoic acid, berberine and more.
b) Ketones and Fasting
The ketone body β-hydroxybutyrate inhibits the NLRP3 inflammasome, providing both anti-inflammatory and neuroprotective effects. Nutritional ketosis or intermittent fasting can be leveraged therapeutically in selected cases (R).
c) Curcumin and Other Nutraceuticals
Curcumin (R), the active compound in turmeric, suppresses NF-κB (an inflammatory molecule) activation and downregulates multiple inflammatory pathways. It has shown benefit across a wide range of inflammatory and autoimmune conditions.
Additional supportive compounds include:
- Resveratrol
- Quercetin
- Glutathione
- Vitamin D
- Magnesium
- And more
- Movement as Medicine
Regular physical activity modulates inflammatory tone through multiple mechanisms:
- Reduction in visceral fat and adipokine release
- Improved insulin sensitivity
- Enhanced lymphatic flow and detoxification
- Upregulation of anti-inflammatory cytokines such as IL-10
However, also here, as always, balance is key. And more of a good thing is not always great! Excessive or high-intensity training without recovery may paradoxically increase susceptibility to infections due to temporary immune suppression, contributing to chronic inflammatory triggers (R/, R).
- Support the Gut-Immune Axis
As elaborated on earlier, gut permeability, dysbiosis, and resulting immune activation are common contributors to systemic inflammation. Addressing this axis may include:
- Probiotic and prebiotic supplementation – only where bacterial levels are too low. Note: In individuals with SIBO (small intestinal bacterial overgrowth), adding in more pre- and probiotics often worsen symptoms and inflammation!
- Eliminating any overgrowing, pathogenic bacteria, yeast or parasites
- Elimination of food sensitivities and reactive proteins
- Repairing gut barrier integrity
- Vagal Nerve and Parasympathetic Activation
Restoring balance to the autonomic nervous system reduces inflammatory tone. Evidence-based techniques include:
- Breathwork and meditation
- Singing, humming, or gargling (to activate the vagus nerve)
- Cold exposure/ saunas
- Sunlight and nature
- Acupuncture and vagus nerve stimulation
These interventions improve heart rate variability (HRV) and modulate the anti-inflammatory reflex.
In Summary: Addressing Inflammation is Foundational to Long-Term Health
While chronic inflammation may be silent, its effects are profound and far-reaching. It is the common thread running through fatigue, brain fog, metabolic dysfunction, cardiovascular risk, and premature aging.
Fortunately, through advanced testing and a personalised root-cause approach, this driver of chronic disease can be modulated.
