Apple-Shaped Body? Real Reasons Your Belly Fat Won’t Budge

In today’s world, one of the most common ‘vain’ concerns people face is the stubborn belly fat that defines the ‘apple-shaped’ body. You know the look: while legs and arms might be thin, there is that persistent muffin top or bloated belly that just won’t budge, no matter how much you diet or exercise.

But what many don’t know, is that this belly fat, if excessive, isn’t just a vanity issue—it’s a signal from your body that should be taken serious, and addressed, if longevity and health optimisation is your goal.

In fact, research suggests that belly fat, in particular if the person is overall skinny (aka ‘skinny fat’), is often a sign of 3 underlying imbalances:

 

  1. Inflammation
  2. Blood sugar issues and insulin resistance
  3. Stress and cortisol imbalances

 

When the body is either inflamed, or struggling with blood sugar or cortisol imbalances (or, which is usually the case, a combination of the three!), our brain starts to send signals to store fat around the waist, and ‘redistribute’ from the legs and arms towards the trunk.

This often means that you likely aren’t overeating (which would normally lead to being overweight or at least heavier overall, not just on the belly, while legs and arms stay skinny), but that your biochemistry is messing with your metabolism and creating that dreaded bulge around the waist.

In today’s blog post I will explain a little more about inflammation, blood sugar control and our daily cortisol (stress hormone) rhythm and the reasons they might be thrown off.

Why Belly Fat Matters More Than You Think

But belly fat isn’t just inconvenient—it in itself is dangerous. Visceral fat that accumulates around your waist is linked to serious long-term health risks including:

  • Type 2 diabetes
  • Cardiovascular disease
  • Alzheimer’s
  • Hormonal imbalances like PCOS and perimenopause symptoms

 

Additionally, the underlying root causes, inflammation, sugar and cortisol imbalances, have been linked to all kinds of health issues, from anxiety, bloating, acne and joint aches, to more serious long term complications such as type 2 diabetes, Alzheimer’s disease, cardiovascular disease, cancers, and more.

This is not about chasing a size zero—it’s about unlocking vibrant health, hormonal harmony, and longevity.

Root Cause #1: Chronic Inflammation—The Hidden Saboteur

 

First things first. Inflammation is not all bad! In fact, it is the body’s natural defense mechanism, and we need it to survive.

When we are injured or fighting off infections, our immune system creates inflammation to help us heal.

But here’s the catch: when inflammation becomes chronic, it slowly erodes your health from the inside out. Think of it as a slow-burning fire—one that doesn’t heal but harms.

 

Early Signs of Chronic Inflammation:

  • Acne, eczema, or skin irritation
  • Irregular periods or hormonal imbalances (PCOS, endometriosis)
  • Joint pain and body aches
  • Anxiety or brain fog
  • Poor sleep quality
  • Trouble sleeping
  • Bloating and belly fat, while being overall skinny, ie ‘skinny fat’

 

The Inflammation–Belly Fat Connection

Inflammation changes the way your brain signals for fat storage. When you are inflamed, the brain tells your body to store fat around your waist—even if you are eating well, exercising, and doing ‘all the right things’. I commonly see this in my health conscious clients (too book a one-on-one consultation with me either in person in Kensington, London, or worldwide on zoom to come up with a bespoke health strategy, email my team at hello@mprecisionhealth).

They are trying their best, yet that layer of belly fat won’t budge. As always within Functional Medicine, we want to know ‘what is the underlying root cause?’ – in this case, ‘Why is there chronic inflammation’?

 

Common Triggers of Inflammation:

  • Gut infections like parasites, candida, or bacterial overgrowth (such as H. pylori, SIBO, and others)
  • Other gut microbiome imbalances like not having enough of the ‘good bugs’
  • Hidden food intolerances (yes, these can often be to ‘healthy; foods! – but if your body has classified them as an ‘invader’, it will create inflammation every time you eat that ‘healthy’ food. Common ones include eggs, gluten, cow dairy, almonds and yeast. Best is always to TEST, not to GUESS – else the diet can become overly, and often unnecessarily restrictive! Not everyone is intolerant to gluten or cow dairy, for example!)
  • Environmental mold or other toxins
  • Blood sugar spikes
  • Stress and sleep issues
  • Deficiencies in nutrient needed to quench inflammation, such as omega 3s.

 

Expert Tip: Even if you don’t have digestive symptoms, gut dysfunction could be the root cause of your apple shape. Functional gut testing often reveals hidden gut microbiome imbalances in my patients that while not presenting with gut symptoms, struggle from systemic inflammation!  

Root Cause #2: Blood Sugar Imbalance—The Silent Fat Promoter

You don’t have to eat donuts every day and be overweight to have blood sugar issues. In fact, many of my patients who are skinny, still deal with the metabolic rollercoaster that leads to stubborn belly fat – and health concerns down the line.

 

The “Hangry” Cycle and Insulin Resistance

 

Ever felt ‘hangry’, ie that hungry that you are angry? If so, you aren’t alone. Many know that feeling. In fact, it is that common, that most people constantly carry snacks around with them, because they feel they constantly  ‘need to eat’ – or else…

But it doesn’t have to be that way – and even, shouldn’t be that way. The just described scenario is a sign you had a blood sugar spike, followed by a dip, making your brain think you are starving and need to eat now!

Here is what happens:

  1. You eat something sugary or high-carb.
  2. Your blood sugar spikes.
  3. It crashes—hard.
  4. You get hangry, tired, and crave more sugar.
  5. Your body starts to become insulin resistant over time (the cells stop responding to the sugar regulating hormone insulin as well any more).

 

In the short term, these blood sugar spikes cause microdamage to our body, and in the long run, can contribute to Alzheimer’s, cardiovascular disease, cancers, and … you guessed it, belly fat accumulation.

Some of the ‘acute’ effects of blood sugar spikes include:

  • Wrinkles and cellular ‘rusting’ via a process called ‘glycation’, where sugar attaches to protein and fats inside the body (and most of our body is made of those 2!), forming ‘advanced glycation end products’, short ‘AGEs’, literally ‘aging’ us!

 

Insulin resistance = weight gain around your waist.

Even if your overall calorie intake is low, these spikes and dips wreak havoc on your metabolism and can signal your body to hoard fat around your belly.

 

Long-Term Effects of Blood Sugar Imbalances:

  • Cardiovascular disease
  • Alzheimer’s (‘Type 3 Diabetes’)
  • Premature aging (wrinkles, joint pain)
  • Mood swings and fatigue
  • Hormonal imbalances and infertility

 

 

Expert Tip: Many “skinny fat” individuals consume small amounts of sugar regularly—just enough to keep them on the blood sugar rollercoaster. The secret isn’t just cutting sugar; it’s stabilizing your levels strategically.

Root Cause #3: Stress and Cortisol—The Hormonal Trigger You Can’t Ignore

 

Stress is not just emotional—it’s biochemical.

Your body doesn’t know the difference between:

  • A tiger chasing you
  • Your inbox overflowing
  • Inflammation in your gut
  • Mold in your home
  • Ruminating thoughts about a stressful conversation
  • Unresolved childhood trauma keeping you hyper alert

 

They all trigger the same biochemical cascade: your adrenal glands release cortisol, the stress hormone, that, if released at the wrong time of the day, signals your body to store fat—again, right around your waist.

 

The Cortisol Curve and The Circadian Rhythm: What You Need to Know

 

Your cortisol should spike in the morning and drop steadily throughout the day. But in most people today, it is either:

  • Too high all day (wired)
  • Too low in the morning, then surges at night (causing sleep issues, wired yet tired)
  • Flatlined due to long-term burning the candle on both ends

 

Each of these faulty patterns leads to:

  • Fat storage around the midline
  • Low energy
  • Immune dysfunction
  • Insulin resistance
  • Increased risk of autoimmunity and cancer

 

Expert Tip: Stress doesn’t just ruin your mood—it literally makes healthy eating much more difficult. When cortisol is off, your body goes into survival mode, making you crave junk food. It switches our control center from the prefrontal cortex, our rational decision making part of the brain, that tells us to eat broccoli and chicken or tofu, to the survival part of the brain, that doesn’t care about our weight or health, only that we need to survive – making us crave highly palatable, high caloric/ fatty and sugary foods.

Having an imbalanced circadian rhythm and cortisol curve also often makes it harder to sleep at night. And if we don’t get the right amount and good quality of sleep, our body becomes slightly inflamed and insulin resistant the next day! Yes, you read that right. After a bad night’s sleep, you might be eating the same food you did a different day, but the body now stores it

The Vicious Cycle: How All Three Root Causes Work Together

Here’s the kicker: inflammation, blood sugar imbalance, and cortisol dysfunction are interconnected.

  • Stress increases blood sugar and inflammation.
  • Inflammation messes with insulin and cortisol.
  • Blood sugar issues fuel inflammation and stress.

 

This trifecta keeps your body in fat-storage mode 24/7—even if you’re doing everything “right.”
